When describing the severity of internal hemroids physicians use a grading scale:
- First-degree hemroids: hemroids which do not prolapse however they can bleed.
- Second-degree hemroids: hemroids that prolapse and then retract on their own. Bleeding at this level may or may not happen.
- Third-degree hemroids: hemroids which prolapse and do not retract however can be pushed back in manually.
- Fourth-degree hemroids: hemroids which prolapse and cannot be pushed back in manually. This type of hemroids can also include hemroids that contain blood clots or pull much of the rectum lining through the anus. These must always be treated by a physician.
Internal hemroids have different symptoms than external hemroids. External hemroids can be felt as bulges at the anus, but they typically display few of the same symptoms of internal hemroids.
One reason may be because they are situated lower in the anal canal thus having little effect on the function of the anus, in particular the anal sphincter. Problems may occur in external hemroids when blood clots inside them. This is referred to as thrombosis. Thrombosis of an external hemorrhoid causes an anal lump that is very painful as they irritate the somatic nerves and often will require medical attention. This type of hemroids may heal with some scarring and leave a tag of skin protruding from the anus. This tag if large can make anal hygiene difficult and irritate the anus.
First through third degree hemroids can be treated through over-the-counter medicines as well as surgery but you should also consider some non-traditional methods.
